Error #481, Invalid Picture when Inserting an Image into a Remark Web Survey Form

Check the color format of your image. If the image is in CMYK format, which is designed for use in print rather than in a browser, it often cannot be displayed on an HTML (web) form.

The bottom line is that RGB (Red Green Blue) is colored light. CMYK are pigments (CMYK stands for Cyan, Magenta, Yellow, blacK) intended for print, which is how color is perceived by reflected light. Since browsers are designed to work with monitors and colored light, there is no implementation to manage the "fourth channel." 
 
As a workaround to this problem, try saving the file using the RGB color format instead of the CMYK format.
