How to Create a Fillable PDF in Remark Office OMR 11

Remark Office OMR 11 provides a feature for you to create a fillable PDF form that you can send out to your participants to fill out electronically.  They would then email the form back to you to read the PDF file into Remark to collect the data.  The fillable PDF is created from your defined template in the template editor.   

Certain things to keep in mind prior to creating a fillable PDF with the template:

  • Although you only need the template file to create a fillable PDF form it is recommended you supply a scanned based PDF file that matches the active template for a higher quality fillable PDF file.
  • For areas on the form where participants will be able to type in their information such as name, there must be a corresponding image region to create the fillable portion on the PDF. 

(Note:  Once the fillable PDF is generated your template can be modified to replace and/or add an OCR region over top of the image regions to capture the typewritten information.)

  • If you are using OMR regions that allow participants to fill in more than one bubble response for a question you must set the region in the template to accept more than one response.  In the Advanced Region Properties and the multiple response setting select the number to allow. 
    • For example, if you have a test form and the participant can fill in more than one answer then you want to set the number to allow all.  (Note: after the PDF is generated the template can be modified to set the OMR region multiples responses back to “Not allowed”.  The allowable responses would then be set by the answer key when graded.) 
    • Another example, if your survey allows for more than one response to be selected then you must set that in the template in Advanced Region Properties under “multiple responses” and the template would not need to be modified after generating the PDF.


To create a fillable PDF

1. Create your template file as you normally would.

2. In the Remark Office OMR Template Editor, select the File menu and click Create Fillable PDF.


3. If you have an actual PDF file of the form, select the ellipse next to the box Base PDF and browse to the location of the PDF file. Supplying the base PDF creates a better-quality form.

4. Select Open the fillable PDF file after saving.

   (Note:  The PDF file should be a scanned image of your form scanned at 300 dpi in Black and white.)


5. Click the Save button, choose a location for the PDF file, and then click Save.


We recommend testing out your PDF to ensure it works as you expect. You can then distribute the PDFs to your recipients in the manner that works best for you (e.g. email, on a website, etc).
